 Dress Code - white or club shirt above the waist and grey below the waist (white for semi final & final).

Cheques should be made payable to CLEVEDON BOWLS TOURNAMENT.  Please enclose a sae if a receipt is required.  This
acknowledges receipt only but does not guarantee entry.
Each entry form together with the total remittance must be enclosed in its own individual envelope.  This should be handed in to the 
Committee at the Clevedon or Clevedon Promenade Bowling Club during the Tournament week or posted to the Secretary,  
Clevedon Bowls Tournament, Clevedon Bowling Club, Chapel Hill, Clevedon, BS21 7LL.
Entries will be recorded in order of receipt and any unsuccessful applicants will have their fees returned
If you have to withdraw from an event please try to give the Tournament Secretary at least 48 hours notice.  No entry fee can be
refunded under any circumstances.  CLOSING DATE FOR ENTRIES 30th April 2011.
The tourmament draw will be available for viewing from early June at either www.clevedonbowlingclub.co.uk or www.clevedonprom.org.uk
In compliance with the Child Protection Act, if any applicant is under 18 years old, or is a vulnerable person, please ensure that you
bring a consent form signed by the parent or guardian.  Your Club CP Officer will provide the necessary form, alternatively please
contact the Tournament Secretary.
